Dear Parents,
Due to our recent legislation AZ House Bill 2161, parents must be able to review a survey and provide consent at least 7 days prior to their child completing a survey .
We have provided you with the link to the survey as well as the parental consent form which must be signed and turned in prior to your child receiving the survey.
7th-12th grade: SVUSD Parent Consent – Climate Survey Middle/High
We will be sending home a paper version of the consent form when we return from fall break. Please sign and return to your child's teacher if you give parental consent.
